As a consequence of the aging population, the number of people who have multiple chronic diseases and comorbidities is swelling. In conjunction with the Rheumatology Unit at the Royal Adelaide Hospital the Chronic Disease Management system was developed, powered by Miya’s DSE.

While some chronic disease management tools implement care planning, the Chronic Disease Management system goes further in actively assessing disease activity and treatment toxicity while automatically applying complex decision support to assist in patient management; managing multiple high risk drugs is a timely issue.

The Chronic Disease Management system incorporates full support for care plans, guidelines, assessments, medication management, electronic orders and monitoring of patients between visits.
