Our Miya Assessments documentation tools, previously available in Patientrack, address a critical factor in risk screening. They provide a standardised framework that allows key criteria to be captured and monitored. They are aligned with disease, care processes, service and other parameters to ensure that the appropriate data is recorded at the right time.

Miya Assessments is defined using a form builder tool that allows our customers to configure assessments aligned with their local practice and based on accepted standards.
Capturing this information electronically means that the data can be used to actively drive patient care. Information can be recorded at the point of care promoting accuracy and removing the need for transcription.

Assessments can be created to support all clinical roles and documentation requirements such as medical, nursing, allied health and pharmacy. They can be configured as multi-disciplinary seeking input from all members of a care team.
Customers can define a workflow or care plan to be initiated based on a stage of the episode such as admission or the outcome of an assessment, for example “score greater than X”. Whether monitoring for Sepsis or Falls Risk, the resultant data can be used to suggest next steps in that care pathway.
Miya Assessments can be configured using controls such as check boxes, radio buttons, selection lists, image capture, image annotation, and free text. Miya Assessments links to local and external approved references to align practice and enables conditional formatting to mandate collection of the required data set.
For some types of assessment, it is important for the assessor to monitor results over time. These assessments can be configured with the previous responses in context, so trends can be easily reviewed and appropriate action can be taken.
We provide an extensive library of standard assessments as part of the baseline build that customers can use as the starting point for their requirements. These assessments can be adjusted to local practice as needed.
